ESOC震撼发布：急性缺血性卒中静脉溶栓迎来多项证据，破解临床难题！
本期要点
近年来，急性缺血性卒中（ AIS）领域进展显著，替奈普酶作为一种新型纤溶酶原激活物，近年来已成为再灌注治疗领域的研究热点。然而，在特殊人群和特殊场景下，静脉溶栓仍有许多未解决的临床问题。近期举办的2025年ESOC大会围绕临床未满足需求，带来了多项研究成果，以下为亮点速览：
桥接治疗：对于急性前循环大血管闭塞缺血性卒中，一项多中心登记研究显示，原研替奈普酶桥接机械取栓治疗可显著改善3个月功能预后，且这一改善不受预期治疗时间影响；对于急性后循环大血管闭塞缺血性卒中，登记研究和荟萃分析显示，桥接治疗较直接取栓，3个月功能预后更优，且不增加症状性颅内出血发生风险；对基底动脉闭塞（BAO）患者而言，多中心队列研究显示，单独静脉溶栓较取栓联合或不联合静脉溶栓，临床结局更优。
轻型卒中：一项登记研究分析显示，对于轻型卒中患者，相较于非静脉溶栓，静脉溶栓治疗与良好的3个月功能结局显著相关千层金，且两组死亡率无显著差异。ORIGINAL研究亚组分析显示，原研替奈普酶治疗致残性轻型卒中疗效和安全性与阿替普酶相似。
超窗治疗：多项研究支持超时间窗（＞4.5h）静脉溶栓治疗仍可为患者带来获益。
